What to expect at your first ENT appointment
Welcome to Trahan ENT & Aesthetics! We appreciate the opportunity to care for you and your family. Our office is focused on delivering professional, high-quality care to all patients. To better serve you, we are providing you with a few notes about your first visit with us!
- Please print and complete your new patient paperwork. We can mail the paperwork to you as well.
- Please arrive 15 minutes prior to your scheduled appointment time
- We will need a copy of your current insurance cards and drivers license.
- If you have had any testing or imaging, we ask that you bring to your appointment.
- Please bring a list of current medications.
- If the patient is under the age of 18, we ask that a parent/guardian be present.
Upon check-out, please be prepared to pay your co-payment or cost of visit depending on your insurance plan benefits. If you have questions prior to your visit, or need to reschedule, you may contact us at 337-594-6002.
**If you are unable to complete your paperwork prior to arrival, please arrive 30 minutes early for your appointment. We kindly ask that you allow extra time as the office must complete an accurate administrative portion to prepare your chart.
